Despite being the most likely of the four, the most-ordered items score lowest, because a back injury is serious and rarely fatal. Read down the deadline column rather than the score column, and that is the order the work should happen in.
The forklift crossing shows what a control does to a score. A painted walkway and licensed drivers leave likelihood at 3 and severity at 5, so it sits at 15.

Name the technique you used in two lines and move on. A reader needs to know a risk matrix produced the numbers, not how a risk matrix works.
