Incident Reporting
Incomplete and delayed incident reports are one of the most common gaps in workplace safety. This gives workers the fastest path to reporting, with AI converting a voice description into a structured incident record.
The fastest way to report is by speaking. Workers tap the floating voice button, describe what happened, and the AI fills in the form automatically, covering incident type, location, body parts affected, treatment level, and more. Video and photo modes work the same way.
For more structured reporting, a three-step wizard walks through what happened, the detail form, and a risk matrix. Either way, incidents are routed automatically: Severity 1 and 2 trigger a full investigation (Track A), Severity 3 goes to a light investigation (Track B), and Severity 4 and 5 are logged and closed (Track C).
Audit and Inspection
Inspection records are only useful if they reflect exactly what was checked and when. Our EHS platform makes every inspection traceable, scored, and locked to the checklist used at the time.
Templates support conditional logic, scored answer sets, and version control. Each published version is locked so inspections always reference the exact checklist used. Completed inspections generate a scored Pass, Needs Attention, or Fail report automatically based on the percentage of questions answered correctly.
Investigation and Root Cause Analysis
Most repeat incidents trace back to investigations that were incomplete or never properly closed. Our EHS software connects every serious incident to a structured workflow with built-in accountability at every step.
For Track A and B incidents, a linked investigation record opens automatically. Lead investigators get a structured workspace for 5-Why root cause analysis, findings, evidence, and team management. Corrective and preventive actions are assigned directly from the investigation, with an independent verifier required. The same person cannot both complete and sign off on a corrective action.
Risk Management
Hazards that are identified but never formally scored rarely get resolved. Our software gives EHS teams a structured register to document, score, and move every risk through a defined workflow.
Teams can register hazards, score inherent and residual risk on a 5x5 matrix, and record controls across the full hierarchy: Elimination, Substitution, Engineering, Administrative, and PPE. Risks move through a defined workflow from assessment through mitigation, control scoring, and acceptance or closure.
Asset Tracking
Assets disconnected from safety workflows create blind spots in maintenance and incident records. Our platform links assets directly to inspections, incidents, and maintenance schedules so nothing goes unnoticed.
Assets can be linked to incidents, inspections, and preventive maintenance schedules covering preventive, calibration, inspection, and other recurring tasks. Overdue schedules surface automatically on the dashboard. Maintenance records with a Conditional or Fail outcome can escalate directly to a corrective action.

The OSHA 300 Log updates in real time. OSHA 300A totals calculate automatically and support electronic certification for ITA submission. RIDDOR F2508 filings and SafeWork NSW notifiable incident records are tracked per incident, with phone and written notification logs built in. TRIR, DART, LTIR, and Severity Rate calculate automatically and display on the dashboard. No spreadsheet required.
